An affidavit of discovery made pursuant to Section 93 of the Act shall unless an application form is prescribed that includes the relevant averments, be in Form 16. Unless the Authority permits otherwise, the affidavit shall be made by the applicant. Where made by any person other than the applicant, the deponent shall state his/her means of knowledge. In the case of a corporate body, the affidavit may be made by the Secretary, Law Agent, or by a person duly authorised by that body where it is averred that the person is duly authorised and has the necessary means of knowledge.
